29 CFR 1910.178(l)(6): Certification. The employer shall certify that each operator has been trained and evaluated as required by this paragraph (l). The certification shall include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation: Employees engaged in material handling activities at Kinney Manufacturing, LLC, doing business as IntoMetal, located at 3340 North 33rd Street, Lincoln, Nebraska were exposed to struck-by hazards in that the employer did not certify that each operator was trained and evaluated prior allowing employees to operate the walk-behind power lift.
