29 CFR 1910.147(d)(6): Prior to starting work on machines or equipment the authorized employee did not verify that isolation and de-energization of the machine or equipment had been accomplished. On or about May 23, 2023, at the Prime G Reboiler Pass 4 Control Valve at the Wynnewood Refinery in Wynnewood Oklahoma, employees did not verify the equipment being maintained had been isolated and de-energized. This exposed the employees to fire and chemical release hazards.
