29 CFR 1910.147(e)(2)(i): Before lockout or tagout devices were removed and energy was restored to the machine or equipment, authorized employees did not ensure that all employees had been safely positioned or removed from the work area: (a) On or about February 8, 2023, the employer did not ensure all employees working on press five (5) were safely positioned or removed from the top of the press when running a test run on press five (5) presenting a caught in hazard to the employees.
