29 CFR 1926.1427(d)(4):An employer permitted an operator to operate equipment with a certification card issued more than 5 years prior to the date of operation: On or about January 9, 2023, at the job site, a lattice boom crane operator was permitted to operate the crane with a certification card issued more than 5 years prior to the date of operation. Employees were exposed to struck-by hazards during crane operations.
