29 CFR 1910.253(b)(2)(iv):Valve protection caps, where cylinder is designed to accept a cap, shall always be in place, hand-tight, except when cylinders are in use or connected for use. Employees engaged in maintenance activities at J F O'Neil Packing located at 3120 G Street, Omaha, NE 68107, were exposed explosion hazards in that two acetylene cylinders were not equipped with caps.
