29 CFR 1910.119(f)(1)(iii)(B): The employer did not develop and implement written operating procedures that provided clear instructions for safely conducting activities involved in each covered process consistent with the process safety information that addressed health and safety considerations, such as precautions necessary to prevent exposure, including engineering controls, administrative controls, and personal protective equipment. (a) Room 30 HEPES process - On or about 11/9/2022, the employer operates and maintains a covered process involving methanol, a class 1 flammable liquid having a flashpoint below 100F exceeding the TQ of 10,000 lbs. The employer did not develop and implement written operating procedures, including the SAN WI 235 Loading HEPES Crystallizer T-30.03 with Methanol (effective date 9/01/2022) procedure; that provided clear instructions for safely conducting activities involved in each covered process consistent with the process safety information that addressed health and safety considerations, such as precautions necessary to prevent exposure, including engineering controls, administrative controls, and personal protective equipment.
