29 CFR 1910.147(d)(3): Machine or equipment isolation. All energy isolating devices that were needed to control the energy to the machine or equipment shall be physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s). On or about September 22, 2022, employees were exposed to chemical burns, fire, explosion and toxic gas release hazards when the employer did not ensure the energy isolation block valve on the low pressure side of the flow transmitter CLF615 continued to hold until the maintenance was completed.
