29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to Items of this sec(a)(LOCATION)(IDENTIFY SPECIFIC OPERATIONS AND/OR CONDITIONS)(IDENTIFY MACHINERY OR EQUIPMENT) (a) On or about October 4, 2022, the employer developed machine specific energy control procedures did not identify specific lockout points for each energy source, how to deenergize energy sources, and how each energy source is verified to be deenergized. The specific energy control procedures that did not meet required energy control elements were, but not limited to the following: (1) The employer developed machine specific energy control procedures for the Sort and Feed Conveyor did not identify specific lockout points for each energy source, how to deenergize energy sources, and how each energy source is verified to be deenergized. This exposed employees to caught-in and crushing hazards. (2) The employer developed machine specific energy control procedures for the IPS Baler did not identify specific lockout points for each energy source, how to deenergize energy sources, and how each energy source is verified to be deenergized. This exposed employees to caught-in and crushing hazards. (3) The employer developed machine specific energy control procedures for the LUBO screen sorter did not identify specific lockout points for each energy source, how to deenergize energy sources, and how each energy source is verified to be deenergized. This exposed employees to caught-in and crushing hazards.
