29 CFR 1910.146(e)(1): Before entry was authorized, the employer did not document the completion of measures required by 29 CFR 1910.146(d)(3) by preparing an entry permit: a) DOA Tower Line 1; On or about August 24, 2022 and at times prior, employees were exposed to entrapment and struck-by hazards, in that the employer did not ensure that an entry permit was completed when employees were required to enter the DOA Tower Line 1 to perform weekly cleaning duties. b) Ovens Line 1; On or about August 24, 2022 and at times prior, employees were exposed to entrapment, respiratory, and burn hazards, in that the employer did not ensure that an entry permit was completed when employees were required to enter the ovens on line 1 and line 2 to perform weekly cleaning duties. c) Ovens Line 2; On or about August 24, 2022 and at times prior, employees were exposed to entrapment, respiratory, and burn hazards, in that the employer did not ensure that an entry permit was completed when employees were required to enter the ovens on line 1 and line 2 to perform weekly cleaning duties. d) Dust House; On or about August 24, 2022 and at times prior, employees were exposed to entrapment and struck-by hazards, in that the employer did not ensure that an entry permit was completed when employees were required to enter the dust house to perform weekly maintenance duties.
