29 CFR 1926.652(a)(1):Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). On or about July 25, 2022, an employee was not protected by a protective system while working in a trench at a depth of 5.5 feet, exposing the employee to struck-by hazards and caught-in hazards associated with a cave-in. Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
