29 CFR 1910.146(c)(2): The employer did not inform exposed employees, by posting danger signs or by any other equally effective means, of the existence and location of and the danger posed by permit spaces. (a) 157 West Jackson Street, New Holland PA, roof, condenser #3 - On or about June 9, 2022, the entry to the condenser was not marked with a danger sign or other notice of confined space. Employees were exposed to hazards associated with confined space to include caught in and hazardous environment due to presence of ammonia. (b) 157 West Jackson Street, New Holland PA, roof, condenser #4 - On or about June 9, 2022, the entry to the condenser was not marked with a danger sign or other notice of confined space as identified on the AFP Confined Space Program. Employees were exposed to hazards associated with confined space to include caught in and hazardous environment due to presence of ammonia. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
