29 CFR 1926.1211(c)(1): The employer did not ensure that retrieval systems or methods were used whenever an authorized entrant entered a permit space, and did not confirm prior to entry that emergency assistance would be available in the event that non-entry rescue failed: a) At the jobsite, where employees entered storm water lines, retrieval systems were not used during entry, exposing employees to inhalation and engulfment hazards.
