29 CFR 1910.146(d)(3):Under the permit-required confined space program required by 29 CFR 1910.146(c)(4), the employer did not develop and implement the means, procedures, and practices necessary for safe permit space entry operations. a. Blast - On or about January 20, 2022, employees were exposed to caught in hazard from entry into permit required confined spaces without having developed procedures or safe practices. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
