29 CFR 1910.253(e)(5)(iv): Hose connections were not clamped or otherwise securely fastened in a manner that would withstand, without leakage, twice the pressure to which they would normally be subjected in service, but in no case less than a pressure of 300 psi. On or about December 29, 2021 and at times prior thereto, at the yard, an employee was using a cutting torch to cut metal and the hose connections were not securely fastened and were leaking. This exposed the employee to a fire hazard.
