29 CFR 1910.147(c)(6)(i):The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: (a) Entire Facility: On or about 10/06/2021, and at times prior, the employer exposed employees to thermal, electrical, struck-by and caught-in hazards, in that the employer did not conduct a review of the LOTO procedures for employees working on equipment such as, but not limited to, Galiso Recertification Equipment: (Galiso Wet Jacket -Model# RSP-1DA, S/N RSP-1202-0223A; Galiso Direct Expansion- Model# RSP-1DA-DE, S/N RSP 1202-0224A; Inverter; compressor; etc.), on an annual basis.
