29 CFR 1926.602(c)(1)(viii)(A): The safety platform was not firmly secured to the lifting carriage and/or forks: a) Jobsite located at 2001 Beach Street, Philadelphia, PA- The employer did not ensure that the job-made platform used to elevate employees was firmly secured to a JLG all-terrain forklift. Employee was working from the job-made platform which was elevated approximately 30 feet from the ground; thus exposing the employee to fall hazards. Condition was observed on or about September 8, 2021.
