29 CFR 1910.146(c)(4): When the employer decided that its employees would enter permit spaces, the employer did not develop and implement a written permit space entry program that complied with 29 CFR 1910.146: (a) Production Area; On or about June 25, 2021, and times prior to, the employer had not developed a site specific written confined space program for employees required to enter confined spaces, such as, but not limited to the paint oven room and/or the 600 gallon mixing tank exposing employees to chemical inhalation and oxygen deficient hazards.
