29 CFR 1910.140(c)(22): Personal fall protection systems were not worn with the attachment point of the body harness located in the center of the employee's back near shoulder level. a) Throughout the warehouse, on or about July 1, 2021, the employer failed to ensure employees were provided properly fitted body harnesses for use with powered industrial trucks (order pickers), thereby exposing employees to hazards associated with falls up to 30 feet.
