29 CFR 1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Q11 Machine: On or about April 29, 2021, and at times prior, employees were exposed to caught in hazards while performing daily operations utilizing the Q11 machine without a guard affixed to the front of the machine. b) Q11 Machine: On or about April 29, 2021, and at times prior, employees were exposed to caught in hazards while adjusting controls on the Q11 machine through an access door.
