29 CFR 1926.651(i)(3): Sidewalks, pavements, or appurtenant structures had been undermined and a support system or another method was not provided to protect employees from the possible collapse of such structures: (a) On or about April 6, 2021, at a job site located West of Sentry Drive on West Sunset Drive, Waukesha, WI, employees working in and around the 15 foot deep, by 100 foot long trench excavation were exposed to struck by, crushing and fall hazards associated with undermined portions of the concrete road directly adjacent to the south side of the excavation.
