29 CFR 1910.215(b)(9):The distance between the abrasive wheel periphery and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one-fourth inch (a) In the southwest side of the ammonia compressor room, on or about 3/23/2021, the tongue guard on a Westward 4TM70 6" Bench Grinder was not adjusted to within one-fourth of an inch, exposing employees to projectile hazards from flying wheel fragments.
