29 CFR 1910.253(d)(1)(ii)(C): A pressure regulating device was not used at each station outlet when the connected equipment was used at pressure less than 250 psig (1.7 MPa): On or about March 31, 2021 and at times prior, at the yard for torch cutting metal, pressure regulating devices were not used at 3 of 6 station outlets of the oxygen manifold where cutting torch hoses were connected and the supplied oxygen pressure was less than 250 psig. Employees were exposed to fire hazards during torch cutting operations.
