29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: On or about March 11, 2021, at the electrical control room for the metal sorting machines, the employer did not ensure that each employee, who worked at an inclined conveyor to replace a conveyor belt, affixed a personal lockout device on the energy isolating device. Employees who repaired the conveyors were exposed to caught-by hazards.
