29 CFR 1926.652(d)(1): Materials and equipment used for protective systems were not free from damage or defects that might impair their proper function: a) Northwest of Intersection of Voshell Mill Road and East Street, Camden, DE 19934: On or about 03/10/2021, employees engaged in sewer installation activities in an excavation where the support system was used with damaged components and defects including but not limited to damaged spreaders, missing parts (spreaders), and pins which were not appropriate for use with the system according to manufacturer's data.
