29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not conducted at least once every three years: Employees were exposed to struck-by hazards. Employees engaged in material handing activities using the Clark and Nissan forklifts, were exposed to struck-by hazards in that an evaluation of each powered industrial truck operator's performance was not conducted: a) An employee operated forklifts while the person's performance had not been evaluated since 2/25/2016. b) An employee operated forklifts while the person's performance had not been evaluated since 9/12/2013. c) An employee operated forklifts while the person's performance had not been evaluated since 8/4/2011. 29 CFR 1903.19(c)(1) requires certification that the abatement of the above violation is complete.
