29 CFR 1910.147(d)(5)(i): Following the application of lockout or tagout devices to energy isolating devices, all potentially hazardous stored or residual energy shall be relieved, disconnected, restrained, and otherwise rendered safe. a) On or about December 10, 2020, after the application of lockout or tagout devices the employer had not ensured all potentially hazardous stored or residual energy had been relieved, disconnected, restrained, and otherwise rendered safe for authorized employees such as but not limited to operators and maintenance personnel repairing and performing preventive maintenance on the Rittal 2009 Riweld Seam Welder machine number 21041405 in that: 1. Residual for the pneumatic air at approximately 100 P.S.I. was not removed from the system. 2. The power terminals for the torch movements on tower 1 and 2 (AKA stationary and moving towers) are live for up to 3 minutes after powering off, and terminals are not assured to be disconnected.
