29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D): On or about 3/5/2020, at 11388 GA-17, Lavonia, GA: Employees performing service and maintenance on equipment used to manufacture truss system were exposed to amputation and caught-in hazards, in that procedures developed did not clearly and specifically outline scope, purpose, authorization, rules, and techniques to control the pneumatic pressure, rotating, moving, gravitational, and mechanical energy sources. a. Upipoint Saw with electrical 480 volts and pneumatic 120 psi energy sources. b. Small Floor Table with electrical 480 volts and pneumatic 120 psi energy sources. c. Compressors two (2) each with electrical from 120 to 480 volts and pneumatic 120 psi energy sources. d. Truss Tables six (6) each with electrical 480 volts and pneumatic 120-psi energy sources. e. Various Saws eight (8) each with electrical from 120 to 480 volts energy source.
