29 CFR 1926.652(a)(1): Each employee in an excavation shall be protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section. a. Located at 9975 Honda Hills Road in Thornville, Ohio: On or about February 17, 2020, the employer did not ensure that a protective system was in place to prevent cave-in hazards for employees performing work in the trench, thereby exposing employees to a crushed by hazard.
