29 CFR 1910.119(j)(5): Equipment deficiencies. The employer did not correct deficiencies in equipment that are outside acceptable limits before further use or in a safe and timely manner when necessary means were taken to assure safe operation. On or around November 3, 2019 and at times prior thereto, the employer at Dow Chemical Company Glycol #2 facility in Plaquemine, Louisiana did not ensure the check valve on the vapor feed line from D-410 (Knockout Drum) to T-410 (Acid Scrubber) remained functional to prevent 5% sulfuric acid solution from back flowing into D-410, which contained liquid ethylene oxide.
