29 CFR 1910.133(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation. a. On or about October 9, 2019, the employer failed to provide and require the use of eye and face protection for the thermal hazards related to oxygen fire and explosion risk associated with transfilling oxygen cylinder tasks. Aircraft service employees utilized a compressed gas system that included a Futurecraft oxygen intensifier without PPE for protection against thermal hazards. On September 17, 2019, an employee suffered serious injuries that included burns, from an oxygen explosion/fire event related to required oxygen transfilling duties.
