29 CFR 1910.178(q)(7): Industrial trucks were not examined before being placed in service. Employees of Beaver Express Services, LLC, working in the dock area, were exposed to struck by hazards when a) Employees were operating Nissan forklifts #104 and #205 that the horns and backup alarm where not operating. b) Employees while operating powered industrial trucks Nissan forklifts #104 and #205 with poor thread on the tires. c) Employees operate Nissan forklifts #104 and #205 without proper restraint device system to reduce entrapment risk.
