29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees. The employer does not ensure lockout/tagout devices are used during service and maintenance of equipment. This violation was observed on or about January 16, 2019, in the facility, an employee was observed conducting maintenance on equipment without using lockout/tagout devices, exposing employees to caught-in and struck-by hazards. Pursuant to 29 CFR 1903.19, within (10) calendar days of the date of this citation, the employer must submit documentation describing the steps taken to ensure the use of lockout/tagout devices while servicing and conducting maintenance on equipment.
