29 CFR 1910.140(d)(2)(ii): The employer did not ensure that personal fall arrest systems were rigged in such as manner that the employee cannot free fall more than 6 feet (1.8 m) or contact a lower level. a. On or about November 30, 2018, at the elevator truck bay, employees were exposed to a fall hazard when the employer did not ensure that the lanyard selected for use was of a length that would prevent contact with the concrete floor.
