29 CFR 1910.334(a)(2)(ii): There was a defect or evidence of damage that could have exposes an employee to injury and the defective or damaged item was not removed from service and an employee was using it before repairs and tests necessary to render the equipment safe were made: a) Small Bird Area - On or about October 5, 2018, the employer failed to ensure that damaged portable electrical equipment was removed from service. Employees, using an overhead-underhung hoist with a pendent Hanyoung Nux HY1022 Hoist Switch with a missing knob/button, were exposed to electrical hazards created by the exposed internal conductors covered with electrical tape.
