29 CFR 1926.21(b)(2): The employer did not ensure that it instructed each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury. This violation occurred on September 6, 2018, when employees were exposed to cave-in hazards with no safe means of egress or cave-in protection. At least two (2) employees worked at the base of a trench that was more than 7 feet in depth.
