29 CFR 1910.178(a)(4): Modifications and additions which affect the capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: (a) VerandaBlue, LLC at 2500 W 4th Ave, Denver, CO 80219: On and before August 24, 2018, the employer modified a powered industrial truck in a manner which affected its capacity and safe operation without the manufacturer's prior written approval. The employer allowed employees to connect a pallet to a forklift's tines, and to connect a personal fall arrest system to the forklift's mast, in a manner which the employees used to elevate and access storage racks. This condition exposed employees to fall hazards.
