29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket). a) On or about August 14th, 2018 or prior thereto: Strain relief was not provided on a 220 V extension cord with a black plug that was connected to a portable receptacle labeled P22. The cord was supplying a Miller Mig Welder. The cord was not inspected for damage, and removed from the work site prior to use of the Welder. b) On or about August 14th, 2018 or prior thereto: Strain relief was not provided on a 220 V extension cord connected to a portable receptacle labeled P22. The cord was supplying a Miller Mig Welder. The cord was not inspected for damage, and removed from the work site prior to use of the Welder. c) On or about August 14th, 2018 or prior thereto: Strain relief was not provided on a 220 V extension cord with a white plug that was connected to a portable receptacle. The cord was supplying a Miller Mig Welder. The cord was not inspected for damage, and removed from the work site prior to use of the Welder. Note: Abatement certification is required for this item.
