29 CFR 1926.451(g)(4)(ii): The top edge height on all suspended scaffolds where both a guardrail and a personal fall arrest system are required were not between 36 inches (0.9 m) and 45 inches (1.2 m) above the platform surface: a) Jobsite: 3525 I Street, Philadelphia, PA 19134 building Rooftop - Employees conducting masonry work from a two point suspended scaffold on a 5th story window were exposed to fall hazards. The top edge height of the guardrail was 29 1/2 inches above the platform. This was documented to occur on August 8, 2018.
