29 CFR 1926.454(b): The employer did not have each employee who is involved in erecting, disassembling, moving, operating, repairing, maintaining, or inspecting a scaffold trained by a competent person to recognize any hazards associated with the work in question. The training shall include topics listed in paragraphs (1) through (4): The employer is failing to ensure employees who are erecting, disassembling, moving, operating, repairing, maintaining or inspecting a scaffold are trained by a competent person to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. This was most recently documented at 4708 S.48th St. Lincoln, Nebraska on or about July 5, 2018. Employees were working on a platform elevated approximately 14 feet which had multiple issues including but not limited to, partially planked working surface, not mudsills and no standard guard rail system.
