29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: a) On or about June 7, 2018, and at times prior to, the employer did not perform a periodic inspection of the LOTO procedures to ensure equipment was properly deenergized prior to servicing and maintenance. Deficiencies in the procedures included, but were not limited to missing energy sources and magnitudes.
