29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch: On or about 5/22/2018, at the maintenance department of CGI Windows and Doors Inc., 3780 West 104th St, Hialeah, FL. An employee was exposed to a struck by hazard while working with a Palm Green bench grinder, Model No. 9682073, having tongue guards not properly mounted, resulting in gaps exceeding 1/4-inch between wheel and tongue guards on both sides.
