29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven -7 feet or less above floors or platforms were not enclosed: On or about April 17, 2018, at the facility complex located at 6625 W. Wilshire Blvd in Oklahoma City, OK, the chain and sprocket on a bolt polishing machine was not guarded. The employees were exposed to caught-in and amputation hazards.
