29 CFR 1910.305(g)(2)(iii): Flexible cords are not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws: a) The employer, West Side Fabrication, LLC, does not ensure that flexible cords are connected to devices and fittings so that tension would not be transmitted to joints or terminal screws. On or about March 6, 2018 and at times prior, employees were exposed to this hazard while working in the vicinity of the Piranha ironworker multi-conductor cord that enters the panelboard with no device such as a bushing to prevent direct pull at the panel terminations. b) The employer, West Side Fabrication, LLC, does not ensure that flexible cords are connected to devices and fittings so that tension would not be transmitted to joints or terminal screws. On or about March 6, 2018 and at times prior, employees were exposed to this hazard while working in the vicinity of and operating the powered chain hoist without a strain relief device for the pendant. West Side Fabrication, LLC was previously cited for a violation of this Occupational Safety and Health standard which was contained in OSHA Inspection Number 1261379, Citation Number 1, Item Number 2a, and was affirmed as a final order on February 7, 2018, with respect to the same cords at the same workplace located at 2120 Naegele Road, Colorado Springs, CO.
