29 CFR 1926.1431(m)(1): A pre-lift meeting was not held to review the applicable requirements of this section and the procedures that will be followed: (a) On or about March 20, 2018, and at times prior, the employer did not ensure that a pre-lift meeting was held to review the applicable requirements for hoisting personnel. Employees of Apex Rockfall Mitigation, LLC. were exposed to fall hazards in excess of 50 feet while being hoisted on basket drill DR-52 to conduct rock fall mitigation activities. A pre-lift meeting prior to hoisting personnel was not held and the applicable requirements for hoisting personnel were not followed.
