29 CFR 1910.212(a)(2): General requirements for machine guards. Guards shall be affixed to the machine where possible and secured elsewhere if for any reason attachment to the machine is not possible. The guard shall be such that it does not offer an accident hazard in itself. (a) The Voeller Mixer A, Model: CM500 / S/N: 96020001, used for the roof deck application, had unsecured plywood covers placed over the openings around the mixer exposing employees to struck by hazards when adding additives and checking for quality during the mixing activities. (b) The Voeller Mixer B, Model CM650 / S/N: 2840007, had unsecured plywood covers placed over the openings around the mixer exposing employees to struck by hazards when adding colored powder, additives, and checking for quality during the mixing activities.
