29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes (including skylights) by covers: a) Worksite was located at 235 Grand Street, Jersey City, NJ 07302 Employees were exposed to trip hazards while working in close proximity to holes with partial covers approximately fifty two inches by sixteen inches that left 6.5 inch openings in the flooring. Employees were cleaning and working in this area on the northeast and northwest stairs on the fourth floor southwest. Violation was observed on or about December 11th, 2017.
