29 CFR 1910.219(c)(4)(ii): The unused keyway(s) of projecting shaft end(s) were not filled up or covered: a) On or about 11/30/17 on the outside roll grader system; employees who work in and around the roll topper and sizing chain were exposed to caught in hazard when there was an open keyway on the rotating drive shaft of the roll toper, located at waist height on the north side of the work platform.
