29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employees trained, the dates of the training, and the signature of the person who conducted the training or the signature of the employer. This violation was most recently observed on 11/28/17, at a jobsite located at 31 Market Street, Ipswich, MA 01938, where the employer failed to provide verification of training for his employees to recognize fall hazards. Prior violations referred to include a violation of this Occupational Safety and Health Standard which is contained in: William Trahant, JR. Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.503(a)(1), which was contained in OSHA inspection number 987538, citation number 2, item number 1, and was affirmed as a final order on July 29, 2017, with respect to a workplace located at 305 Beach Street, Revere, MA. William Trahant, JR. Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.503(a)(1), which was contained in OSHA inspection number 1003916, citation number 3, item number 1, and was affirmed as a final order on July 27, 2017, with respect to a workplace located at Congress Street, Salem, MA.
