29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s): a) Throughout the production areas and yard around the Fabrication shop, on or about January 24, 2017 and at times prior to, employees were exposed to a hazard of being struck by the Hyundai 30L-7A forklift that did not have functioning head lights for use during the night and early morning shifts while working inside the Fabrication shop.
