29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee did not affix a personal lockout or tagout device to the group lockout device before working on the machine or equipment: Water house area - On or about August 28, 2017, supervisor(s), lead(s) and maintenance were required to work together to remove a jam (25 trays or more) inside the machine. Each authorized employee (a supervisor and a lead) did not affix a personal lockout or tagout device to the group lockout device (the new key system) before working on the machine or equipment. Employees were thereby exposed to the hazards of inadvertent start-up of the equipment.
